Keep track of your family's medical history, set health reminders, and review when filling out hospital forms or talking to your health care provider. An accurate medical history helps you and your health care provider maintain your health wellness. If you move or change providers, your records are still with you and this might prevent miscommunication. All medical history information is stored only on your personal device and you control how you share this information. The app does not require an account to be created, and your personal information is not stored on a website. Based on your medical history, a personalized list of recommended health reminders is created. You can add these reminders to any calendar configured in your phone. You can create a reminder on a family-shared calendar to remind family members of appointments. The summary page provides a quick inspection of your medical history that may be useful while talking to your health care provider.

Requires iOS 4.0 or later. Compatible with iPhone, iPad, and iPod touch.
